A Clinical Review of Choledochal Cysts

Abstract
After the first clinical description of choledochal cyst by Alonso-Lej, the conceptions about choledochal cyst has been changed. Now most surgeons favor total excision of choledochal cyst for preventing many complications. This study was divided
into a
drainage group and a excision group, and then they were compared to each other.
A retrospective chart review was made for all patients undergoing operations for choledochal cyst at our institute from January, 1980 to December, 1995.
Forty-two patients underwent various operations for choledochal cyst. Twenty-three total excision procedures(group I) and nineteen drainage procedures(group II) were performed. Before 1988, we mainly performed the drainage procedure, but after
1988,
total excision including trisegmentectomy and Whipple's operations were mainly done Early postoperative mortality was 10.5% in group I, and 4.3% in group II. Cancer change was diagnosed in 3 cases(7.1%) including 1 case of type I which had been
done by
drainage procedure 12 years ago.
Cyst excision is preferred over drainage procedure because it eliminates a reservoir for bile stasis and the possibility of bile duct cancer. The results of the total excision group were excellent, although preoperative conditions of the drainage
group
were worse.
